- Annual maximum. Most plans cap what they pay each year. A large implant case can reach that cap quickly.
- Implant coverage itself. Some plans cover the implant post; others cover only the crown, or treat implants as an alternative to a bridge and pay at that level.
- Waiting periods. Major services may not be covered until you have been on the plan for a set time.
- Missing-tooth clauses. Some policies exclude replacing a tooth lost before your coverage began.

If you have HPSM Medi-Cal dental

It is important to be clear about what that means for implants: Medi-Cal dental benefits cover implants only in limited circumstances, and they generally require prior approval. Phasing treatment: a practical example
When cost is the main constraint, spreading care over time is often the most sensible approach. A typical sequence might look like this:
- First, stabilise. Treat pain, infection and active gum disease. These are the steps that protect the rest of your teeth, and they are usually the best covered.
- Next, prepare the site. Remove the failing tooth and, if needed, place a bone graft. A graft needs several months to mature, which creates a natural pause.
- Then, place the implant. This can fall in a new benefit year, so a fresh annual maximum applies.
- Finally, restore. The crown is fitted once the implant has integrated.

Keeping the option open
If an implant is not possible right now, a few choices keep it possible later. Preserving bone at the time of extraction can make a future implant simpler.
